Maximum Community Support Services, Inc.’s Transition Services offer financial support for eligible expenses associated with an individual’s relocation from an institutional setting to either a group home or a private residence within the community. This funding also applies to transitions from a community residential provider to a private residence within the community. The individual or their legal representative assumes responsibility for these allowable expenses.
